Where can I get a copy of my W-9 and Letter of Indemnification forms?

You can view and download a copy of your W-9 and LOI forms in your MyCast&Crew portal.

1. Log in to your MyCast&Crew account.
2. Click the Loan-out tab in the top navigation bar.
3. You can view and download the most current W-9 and LOI forms in the "Loan-out Information" section.
4. All previously completed documents will be shown on the right side. 
5. The documents are separated by "W-9s" and "Letters of Indemnification." Click the tabs to navigate between the two sections.
   
6. Click the eye icon to view the document. To download, click the down arrow icon.
