ETC Employees: Complete Additional Documents

1. Under the "My Forms" page, you’ll see any documents that need to be completed.
   
2. Any documents listed under the “MyC&C Forms” column need to be completed and submitted in MyCast&Crew. Click Submit in the form’s row to be redirected to http://www.mycastandcrew.com.
   
   
3. Click View under the “ETC Forms” column to fill out a form directly in ETC.
4. Review all information > Check the “Under penalties of perjury…” statement > Add your name in the “Signature” field > Click I Agree. Sign Document and Save.
   
   
5. When you are done filling out a form, the form’s status will change to Complete.
