You will first need to know which entity you were paid by. To find out which entity you were paid by, review your paystubs. On the top corner you will see a company logo:

Employees must report holiday payouts to unemployment.

You may list the department head/supervisor of your most recent production in the supervisor field of the unemployment claim if necessary.
