      Are you looking for a Separation Letter?

      • Your full legal first and last name. 
      • The first and last dates worked in the current year. 
      • Payroll company (ex. Cast & Crew, Viacom, BTL, Caps). 
      • A phone number where you can be reached. 
      • If this is for unemployment purposes, include the state you are filing in.
